Effective Date:
The date you are to be insured depends on the date the participating unit receives the request.
You must be actively at work on the date the insurance is to take effect, otherwise it will take
effect on the date you return to work.
Exclusions:
Limited benefits are paid for pre-existing conditions, special conditions and disability due to alcoholism,
drug addiction and mental, nervous or emotional disorders, as outlined in the Certificate of Insurance.
No benefits are paid for disabilities due to: intentionally self-inflicted injury; a war or act of war;
or committing a crime or attempt to do so.
When insurance ends:
Your insurance will end the earlier of the date the group policy ends, when insurance ends for your class,
when the City Employees Club of Los Angeles ceases to be a participating unit, the end of the period for
which the last premium has been paid for you, or the date you cease to be actively at work on a full-time
basis, except for a leave of absence.
