Accidental Death & Dismemberment Insurance

What is Accidental Death & Dismemberment Insurance?

It is also known as AD&D pays a benefit for death and dismemberment caused by an accident. Dismemberment is defined as the loss or function of limbs, sight, hearing, etc. It’s different from life insurance since it does not pay for death by illness. Typically, this makes the Club’s AD&D premiums much cheaper than life insurance.

Will the AD&D plan pay the benefit along side Life Insurance if I die?

As long as death was caused by an accident, provisions on both the life insurance and ad&d plans allow them to pay out. Verification and proof of death would be required to make benefit payout determination.

What Else Do I Need to Know?

Effective Date: Coverage will take effect on the date United States Life Insurance Company states in writing. Members and their covered spouses and children must not be hospitalized on the date insurance is to take effect. If hospitalized, insurance will take effect on the day after being discharged. Date Insurance Ends: As long as you remain actively at work, continue to pay premiums when due, and the group policy remains in effect, your coverage will not end. Coverage for your spouse and children will cease when your insurance ends, or when they are no longer eligible, whichever occurs first.

Exclusions/Limitations: Benefits are not payable for loss caused by suicide or intentionally self-inflicted injury; insurrection, war or act of war; physical or mental sickness or treatment of that sickness; while on active duty in the armed forces for more than 30 days; voluntary intake of poison, gas, fumes, unless taken as prescribed by a physician; committing a felony, or an attempt to do so; being intoxicated or under the influence of a narcotic, unless prescribed by a physician; and flight in any type of aircraft in which you are the pilot, licensed or unlicensed, or a member of the crew. This does not apply to city airport mechanics when they certify repairs by flying with the pilot. For this express purpose, the mechanics are not considered part of the crew.

Eligibility: You are eligible to apply for AD&D coverage if you are a member of the City Employees Club of Los Angeles and actively working full-time.
