Short Term Disability Professional Insurance Company (PIC)
Plan pays a fixed monthly amount from $500 to $1,500 for up to 5 years, when you are totally disabled regardless the cause of the Disability: accident, illness, work-related or not.
How Do I Get Started
Send the claim form & L.A. City Employee's Club Authorization For Release of Information Form to you payroll/personnel office and ask that office to complete the "Employer's Statement" portion of the claim form. Please, complete the Claimant's Statement form and the Part A. Then forward the form to your attending physician for the completion of Part B.
Please, return the forms to PIC within 90 days after the termination of the period for which the insurer is liable.

WHEN DOES A CLAIMANT RECEIVE BENEFITS? Approximately between 5 to 14 days after PIC receives the claim form. PIC corresponds directly with members regarding any specific information needed.
DOES DISABILITY INTERFERE WITH OTHER LACEA INSURANCE PLANS? No, Supplemental insurance pays on top of any insurance, even Workers Compensation.
