Philadelphia Cancer

This plan pays a lump sum benefit from $10,000 to $50,000 directly to the insured, regardless of any medical coverage of any other cancer plan. Initial diagnosis of cancer triggers the benefit for any financial need of the insured.

How Do I Get Started

  1. Complete the Request for Benefits & L.A. City Employee's Club Authorization For Release of Information Form

    Please make sure you sign the Medical History Authorization part for allowing Occidental Insurance to request the medical information necessary to approve your claim.
  2. If you have Kaiser Permanente, please complete the Kaiser Authorization for Release of Information Form.
  3. Ask your doctor to complete the Health Insurance Claim part of the form, including the medical records, pathology reports, etc.
  4. Mail the completed forms to:

    City Employees Club of Los Angeles
    Attn: Claims Department
    311 S. Spring St. Ste 1300
    Los Angeles, CA 90013
