Kanawha Cancer

The Kanawha Cancer Plan pays a lump sum benefit from $10,000 to $50,000 directly to the insured, regardless of any medical coverage of any other cancer plan. Initial diagnosis of cancer triggers the benefit for any financial need of the insured.

How Do I Get Started

Complete the Cancer Claim-Individual form, and the Kanawha Authorization of Release of Information

Please make sure you sign the Authorization for allowing Kanawha to request the medical information necessary to approve your claim.
If you have Kaiser Permanente, please complete the Kaiser Authorization for Release of Information Form.
Ask your doctor to complete the Attending Physician’s Report part of the form, including the medical records, pathology reports, etc.
Mail the completed forms to:
