Can I take the plan when I retire or leave City employment?

Yes, simply advise us when you retire so that the premiums can be deducted out of your pension or make other arrangements for payment.

Do I have to first get treatment to get paid or reimbursed for treatment cost?

No, treatment is not required for the plan to pay. The plan pays upon initial diagnosis therefore no need to send receipts for reimbursement.

Can I still apply if I have been diagnosed with Cancer in the past?

No. You have to be cancer-free. That is actually the only medical question asked to be eligible for coverage.

If my family has a Cancer history will I be declined?

No. You will only be declined if you or your family member (if applying)has been diagnosed with cancer. it does not matter if you smoke or live in a smoking household, you can still apply.
